Youth Club Consent Form
We are looking forward to welcoming you back to the YEPS youth clubs from the 11th of September, there will be some changes to allow you to have the best possible experience. There is a maximum number of spaces; so first come first served. You will need a consent form that is available to complete and print below or you can have a paper copy from your youth club. You will also be expected to sign up to the new behaviour guidance on arrival.
